Add Another Horizontal Line

Directly above the </TABLE> tag, insert the following tags:

<TR>

<TD COLSPAN=2><HR></TD>

</TR>

Return to your Web browser and reload the surveyform.html file. You should now see a horizontal line below the check boxes. Click each of the check boxes you just created and verify that clicking them alternately selects and deselect them.
